According to multiple sources, some with personal knowledge of the matter, Ms Jacky Oh!, 32, died in Miami, Fla., days after a now-deleted social media post announced that the star was preparing for a mommy makeover.
The cause of her death and when it occurred in relation to the surgery is currently unknown.
Jacky Oh!, whose real name is Jacklyn Smith, was an actress, model, and entrepreneur-creator of the J Nova Collection line of cosmetics.
Oh was also the longtime partner of ‘Wild ‘N Out’ star D.C. Young Fly, whom she started dating in 2015, and a mother to their three beautiful children.
Social media accounts devoted to cosmetic surgery were first to report on MsJackyOh’s passing.
At 3:00 pm Thursday, SurgeonMade, a plastic surgeon rating and referral service, reported having just received definitive data about the star’s death, and that sources said she had undergone a mommy makeover with Dr. Zach, a cosmetic surgeon in Florida.
The account, with 289,000 followers, had posted suspicions of Oh’s death the day it occurred, on Wednesday, May 31st.
“The only info I have is that she did not pass away during surgery…. Everything occurred post op. An autopsy is underway and will provide more information.”

At 3:30 pm Thursday, The Cosmetic Lane, an aesthetic industry marketing agency, said they, too, had received “official confirmation from someone close to the family,” of the star’s passing.
“MsJackyOh had gone for a Mommy Makeover with Dr Zach in Miami and it’s being speculated that she passed away from heart failure but her official cause of death hasn’t been confirmed,” the agency wrote.
The agency said the doctor had since deleted a collaborative post of Oh and Dr Zach, which included the caption, “Getting ready for my mommy makeover….”

No cause of death or fault has been established in the star’s abrupt passing.
According to three separate sources and a preoperative picture of the doctor and Ms. Oh!, Dr. Zachary Okhah, M.D., of Miami, Florida performed Oh’s mommy makeover, after which she passed away.
Okhah bills himself as a liposuction and Brazilian butt lift specialist and is the founder and owner of PH-1 Miami, a private plastic surgery practice, where the surgery is believed to have taken place.
Surgical Times found no indication that Dr. Zachary Okhah is board-certified by the American Board of Plastic Surgery.
Most “mommy makeovers” do not include a Brazilian butt lift, or BBL, though they can.
There is currently no indication or evidence that Oh! had a Brazilian butt lift, nor that the performing surgeon failed to adhere to the standard of care.
Florida
Patient deaths previously prompted Florida’s Board of Medicine to limit the Brazilian butt lift procedure to no more than 3 BBLs per surgeon per day, due to the physical and mental strain the taxing procedure places on surgeons who perform it.
An Aesthetic Society Advisory, issued the following month in July 2022, suggested surgeons also use Ultrasound while injecting fat into the buttocks.
Advisory authors said the use of ultrasound and limiting providers to 3 procedures per day were their most important safety suggestions.
Commonly-cited statistics, from 2017, suggest the BBL has a death rate as high as about 1-in-3,000, but the study that arrived at that number was subsequently criticized as a less-than-accurate survey, conducted over SurveyMonkey, an online survey software platform.
More recent studies, including one conducted in Florida last year, place the procedure’s mortality rate at 1-in-14,952, believed to be well below the mortality rate of an abdominoplasty (tummy tuck).
“The 2017 survey showed a [BBL] mortality rate due to recent PFEs [pulmonary fat embolisms] of 1 in 3,448, whereas the 2019 survey showed a mortality rate of 1 in 14,952. The latter survey indicates that the mortality for this procedure has reached an acceptable level, similar to that of abdominoplasty (1 in 13,193 mortality).”

Dr. Zach did not immediately respond to a request for comment.
The surgeon had released no statement as of 11:15 am Friday, June 2nd.
